Clinical Pharmacy Team
Haringey GP Federation’s Clinical Pharmacy Team supports GP practices to help improve patients’ health and use of medicines.
The team started in 2018 and has grown in response to patient and practice need. It now includes clinical pharmacists and pharmacy technicians working across Haringey’s GP practices as part of the wider healthcare team.
Clinical pharmacists work with patients to:
- Review and explain medicines
- Support people with long-term conditions
- Monitor medicines that need extra care
- Improve medicine safety
Patients may be offered an appointment with a clinical pharmacist to talk about their medicines and how these support their health.
Results and impact
Patients tell us that pharmacist consultations help them feel more confident and informed about their medicines.
Our work has helped to:
- Improve medicine safety and prescribing quality
- Reduce GP workload
- Prevent avoidable harm, including reducing the risk of stroke
- Improve patients’ understanding of their medicines
- 97% of patients would recommend the service
- 100% say their understanding of medicines improved after a review
