Homeless Health Inclusion Team
What the team does
The team helps people who are experiencing homelessness or rough sleeping in Haringey get better access to healthcare, especially general practice (GP) services. Their aim is to reduce health inequalities by making it easier for people without stable housing to be seen by doctors and get the support they need.
They work with local organisations and services to support people with multiple and complex needs, including mental health issues, housing support and substance misuse.
Our Team
The team includes a Specialist GP, Outreach Nurse, Care Navigator, and a Service Manager.
We also have expertise in Mental Health, Housing Support and Substance Misuse.
