Long Term Conditions
Health Inequalities
About this service
We are an NHS service working on behalf of GP practices located within the east of Haringey to provide free health checks for patients who are:
- of a Black, African, Caribbean, or Turkish, Kurdish, and Turkish Cypriot descent or
- who have not had a recent health check but may be at risk of a long-term condition
Our health check includes a blood test, a urine test, blood pressure checks, and conversations about smoking, alcohol, lifestyle changes, and wellbeing. By doing this health check we can assess your risk for underlying conditions and support you and your GP to get you help.
We also offer holistic appointments with our Wellbeing Advocates who can support you with issues like diet and exercise, provide guidance around housing, employment, accessing the NHS, and connect you with appropriate services.
What are we screening for?
We are checking for undiagnosed long-term conditions. Some of these conditions may not have symptoms, and if left undiagnosed/untreated they could cause a serious impact.
It is important that we identify them and manage them correctly. We check for:
Hypertension
When your blood pressure is raised over a time period. This can put you at risk for a heart attack and stroke. Lifestyle changes and medications are available to manage this condition.
Find more information about blood pressure on the NHS website
Chronic Kidney Disease
When your kidneys stop working effectively over a time period. This sometimes has no symptoms. With a diagnosis, we can help lower the chance of the condition worsening.

Diabetes or Prediabetes
Type 2 diabetes can develop when the level of blood sugar in your system is too high. It can sometimes be prevented or reversed with lifestyle changes.
Find more information about diabetes on the NHS website, or learn more about prediabetes on the Diabetes UK website
Raised Cholesterol
Having a raised cholesterol means that you have too much cholesterol (a fatty substance) in your blood. This can be impacted by lifestyle factors.

Why have I been offered this service?
If you have received a call or text from our service, this is because you have been identified based on factors in your medical record like previous diagnoses, test results, or risk factors including smoking.
Our clinical team has reviewed your medical records. You may have had parts of a health check recently, but if you’ve been invited, it’s because some checks are still needed or need repeating.
Where do we work?
We operate in several locations across Haringey. Our team will confirm a location with you when booking an appointment.
Who do we work with?
This service is delivered in partnership with The Bridge Renewal Trust. We also work with several community organisations across Haringey.
