A Multi-Agency Team for Frail Patients with Multiple Conditions

Supporting you to stay well at home

A service jointly provided by Haringey GP Federation and Whittington Health

 

The Multi Agency Care and Coordination (MACC) Team is here to help adults in Haringey who are living with frailty or multiple long term health conditions.

We help these patients to stay well, stay independent and avoid unnecessary hospital visits whenever possible.

The MACC team is operated as a partnership between Whittington Health, Haringey GP Federation, The Bridge Renewable Trust, Adult Social Care, Mental Health Services and local voluntary organisations. This makes it easier to provide joined‑up, personalised care so you don’t have to explain your care journey every time.

 

Who we support

We help Haringey residents who:

  • are 18 or older and live in Haringey
  • live with frailty or multiple long term conditions
  • have had frequent hospital visits
  • are at risk of falls
  • have dementia affecting daily life
  • have many professionals involved and may benefit from care coordination

How the MACC team helps

Holistic assessments and support to stay independent

Looking at your health, daily activities, mental wellbeing, medications, and social needs. We offer home visits if you are struggling to manage at home, so we can understand your daily environment and provide the right support to stay well and safe.

Personalised care plans 

We listen to what matters to you and help set meaningful goals.

Coordinated care

We bring together your GP, therapists, nurses, social care, mental health and voluntary sector professionals so you don’t have to repeat your story.

Preventing crises

We help spot early signs of deteriorating health and make a plan to keep you well wherever possible.

 

How to access the service

You may be referred by your GP or another health or care professional who is working with you, or a volunteer from your local neighbourhood hub.

  • If we’ve supported you before, you may be able to contact us directly.
  • If another service is more appropriate, we will help you get to the right place.
