Clare Duffy

Clare is the Lead Nurse for north east PCN and has worked in Somerset Gardens GP Practice for over 15years. She started as a practice Nurse and has flourished into an ACP, she is one of the clinical leads for our proactive integrated teams project which provides personalised, coordinated support to improve the health and well-being of those waiting for elective care.

She has a special interest in long term conditions especially diabetes and hypertension, and has developed a recent interest in obesity management.
